Make a Step Change Prize Draw
Terms and Conditions 


 

  1. The Prize Draw is open to all Eligible Participants who comply with these Terms and Conditions.
  2. For the purposes of these Terms and Conditions:
    1. “Active” means that the relevant Vitality Plan (set out under clause 2(c) below) has no unpaid premiums, has not lapsed and has not been cancelled by the Eligible Participant or by Vitality;
    2. “Draw Date” means 26 November 2022;
    3. “Eligible Participant ”means any individual who: 
      1. is 18 years old or older;
      2. holds an Active:
        1. VitalityHealth Plan including the Business Healthcare, Corporate Healthcare and Personal Healthcare Plans;
        2. VitalityLife Plan including Personal Protection Plans and Business Protection Plans;
        3. Vitality at Work Plan; or
        4. VitalityInvest Plan;
      3. has registered on Member Zone, and Linked a Device;
      4. is not an employee of Tesco PLC; and
      5. is not an employee of Vitality;
    4. “Linked a Device” means the Eligible Participant has connected a Vitality approved activity tracking device (as listed on Member Zone) to their Member Zone account;
    5. “Member Zone” means the website found at https://member.vitality.co.uk/;
    6. “Prize” means five (5) £500 e-Gift Card, a maximum of one (1) £500 e-Gift Card will be awarded to each prize Winner, as set out further under clause 8;  
    7. “Prize Draw” means the random selection of five (5) Winners of the Prize on the Draw Date in accordance with clause 7;
    8. “Prize Draw Period” means 00.01 24 October 2022 to 23.59 21 November 2022 (inclusive); 
    9. “Vitality” means Vitality Corporate Services Limited and is registered in England and Wales with company number 05933141. The registered office address is 3 More London Riverside, London, SE1 2AQ, trading as VitalityHealth, VitalityInvest, VitalityLife and VitalityCar;
    10. “Vitality Activity Point(s)” means the points Vitality awards to Vitality members under the Vitality Scheme when they perform certain prescribed activities;
    11. “Vitality at Work Plan” means a programme administered by Vitality Healthy Workplace Limited providing workplace health benefits to eligible businesses;
    12. “VitalityLife Plan” means a pure protection insurance contract, administered by Vitality trading as VitalityLife; 
    13. “VitalityInvest Plan” means an ISA, JISA or pension product administered by Vitality trading as VitalityInvest; 
    14. “VitalityHealth Plan” means a general insurance contract for the provision of private medical healthcare, administered by Vitality trading as VitalityHealth; and
    15. “Vitality Scheme” means the benefits programme administered by Vitality pursuant to which Vitality members are offered a range of wellness and lifestyle related benefits.
  3. In order to enter this Prize Draw, an Eligible Participant must first earn three (3) Vitality Activity Points during the Prize Draw Period, following which they will be automatically entered into this Prize Draw (“Initial Entry”). 
  4. After the Initial Entry, Eligible Participants will have the chance to earn one (1) additional entry into the Prize Draw per week if they earn twelve (12) or more Vitality Activity Points each week during the Prize Draw Period (“Additional Entries”).  
  5. Each Eligible Participant is allowed a maximum of four (4) Additional Entries into the Prize Draw (not including the Initial Entry). For the avoidance of doubt, if an Eligible Participant earns 12 or more Vitality Activity Points within the same week as their Initial Entry, this will count as an Additional Entry into this Prize Draw. 
  6. The Prize Draw is free to enter and no purchase is necessary. 
  7. On the Draw Date, Vitality will randomly draw the names of five (5) Eligible Participants via a computer generator, who will be deemed the “Winner(s)” of the Prize Draw.
  8. The Prize will vary depending on the Vitality member’s location, based on the home address registered on the Member Zone. For these purposes, the Prize will be:
    1. a John Lewis/Waitrose & Partners e-gift card, for the Winner(s) located in the United Kingdom; or
    2. an Amazon.co.uk Gift Card, for the Winner(s) located in Northern Ireland.
  9. The Winner(s) of the Prize will be subject to the terms and conditions relating to their Prize, as determined by the providers of the Prize, being either John Lewis/Waitrose & Partners or Amazon.co.uk.
  10. The Winner(s) of the Prize will receive their Prize via email to the email address registered on Member Zone within thirty (30) working days from the Draw Date (subject to any delays that are beyond Vitality’s control).
  11. The Winner(s) may start using their Prize upon receipt. Vitality will not be liable for any lost Prizes in the event that the Eligible Participant has changed their contact details and failed to notify Vitality of this change.
  12. When taking part in the Prize Draw, the Eligible Participant acknowledges and consents that their name, email address and telephone number (“Personal Data”) will be made available to Vitality for administrative purposes. All Personal Data of the Eligible Participant will be held and used for the purposes of the Prize Draw by Vitality in accordance with Vitality’s privacy notice (which can be found on https://www.vitality.co.uk/privacy/ or if not, requested from Vitality).
  13. Eligible Participants are deemed to have accepted and agreed to be bound by these Terms and Conditions upon entry. Vitality reserves the right to refuse entry, or refuse to award the Prize to anyone in breach of these Terms and Conditions. 
  14. Any decisions made by Vitality over eligibility or otherwise in relation to this Prize Draw are final, will not be open to dispute and no correspondence will be entered into.
  15. Vitality must either publish or make available information that indicates that a valid award took place. To comply with this obligation Vitality will send the surname and county of the Prize Winner and, if applicable copies of their winning entry, to anyone who request such information by writing to the Marketing department, Vitality Corporate Services Limited, 4th Floor, 70 Gracechurch Street, London, EC3V 0XL. 
  16. Vitality accepts no responsibility for entries lost, or unsuccessfully completed due to a technical fault, technical malfunction, computer hardware or software failure, satellite, network or server failure of any kind.
  17. Vitality is not liable to any person in any way in relation to this Prize Draw, except for any liability that cannot be excluded by law.
  18. Vitality reserves the right to vary, extend or withdraw the Prize Draw at any time, including in the event of fraudulent behaviour by any Eligible Participant. 
  19. Vitality reserves the right to substitute the Prize with another of similar value, in the event that the Prize is unavailable due to circumstances beyond Vitality’s reasonable control. 
  20. Subject to clause 19, the Prize is non-exchangeable, non-transferable and no cash alternative is offered. 
  21. Vitality reserves the right to amend these Terms and Conditions on the provision of written notice or via Vitality’s website.
  22. The Prize Draw and these Terms and Conditions are governed by English law and Eligible Participants agree to submit to the exclusive jurisdiction of English and Welsh courts and tribunals with regard to any dispute or claim arising under these Terms and Conditions.
