Vitality Football Prize Draw
Terms and Conditions 2024

For the purposes of these Terms and Conditions:
1. “ Draw Date ” means 18 November 2024 being the last date on which winners will be notified of their Prize;
2. “ Eligible Participant ” means any Member aged 18 years or over who is a resident in the United Kingdom and who complies with these Terms and Conditions;
3. “ Event ” means one of:
  1. The AFC Bournemouth v Tottenham Hotspur FC match on 5 December 2024 at 8.15pm (kick-off);
  2. The AFC Bournemouth v West Ham United FC match on 15 December 2024 at 2pm (kick-off);
  3. Or the AFC Bournemouth v Crystal Palace FC match on 26 December 2024 at 3pm (kick-off);
    all taking place at Vitality Stadium, Bournemouth;
4. “ Member ” means an individual who holds a valid Vitality Plan at the time of the Prize Draw;
5. “Health Profile” means the voluntary health questionnaire which Members can complete each Vitality Plan year to receive their ‘Vitality age’- a scientific calculation that assesses a Member’s health based on their health risks and lifestyle;
6. “ Prize ” means tickets for the Winner(s) and a guest to attend the Event (a maximum of 14 tickets to be won in total, 2 per Winner;)
7. “ Prize Draw Period ” means 1 November 2024 to midnight on 10 November 2024;
8. “ Vitality Plan ” means means a VitalityHealth Plan and/or a VitalityLife Plan, either collectively or as appropriate;
9. “ VitalityHealth Plan ” means a general insurance contract administered by Vitality;
10. “ VitalityLife Plan ” means a long-term non-investment insurance contract, administered by Vitality;
11. “ Vitality ” means Vitality Corporate Services Limited trading as VitalityLife and VitalityHealth (registered number 05933141) of 3 More London Riverside, London SE1 2AQ;
12. “ Vitality Status ” means the classifications awarded to Members by Vitality when they earn a minimum prescribed number of points on their Vitality Plan as follows;
  1. 0-799 points (“Bronze Status”)
  2. 800-1599 points (“Silver Status”)
  3. 1600-2399 points (“Gold Status”)
  4. 2400+ points (“Platinum Status”).
The Prize Draw is open to all Eligible Participants who comply with these Terms and Conditions and excludes employees of Vitality and any third parties or agencies connected with or involved in the administration of the Prize Draw.
Eligible Participants will receive an email from Vitality regarding the Prize Draw. To enter the Prize Draw, Eligible Participants must register via the entry link in the email where prompted during the Prize Draw Period. Entries received after the Prize Draw Period will not be valid.
The number of entries Eligible Participants will be eligible for will be based on their
Vitality Status as follows:
1. Bronze Status = 1 entry;
2. Silver Status = 2 entries;
3. Gold Status = 3 entries; and
4. Platinum Status = 4 entries.
  1. Eligible Participants who have completed their Health Profile during the Prize Draw Period will gain one (1) extra entry in addition to the entries gained above.
On the Draw Date, Vitality will choose three (3) Eligible Participants at random by a computer process to win the Prize (the “Winner(s)”). Each Winner can win a maximum of 2 Prize tickets for the Winner and a guest.
Vitality will contact the Winner(s) using the email address or telephone number registered on the Winner’s Vitality Plan, up to three times between 12 November 2024 and 18 November 2024.
The Winner(s) must confirm their acceptance of the Prize via phone or email and provide their postal address, if applicable, by midnight on 18 November 2024 in order to claim their Prize.
In the event that Vitality fails to obtain confirmation from the Winner(s); or the Winner(s) refuse the Prize or for whatever reason cannot accept the Prize, such Winner(s) will forfeit the Prize immediately, and an alternative Winner will be drawn and notified in accordance with these Terms and Conditions.
Once confirmed, the Prizes will either be: (i) posted to the address confirmed by the Winner; (ii) sent to the Winner electronically; or (iii) the Winner will be contacted via email with instructions on how to pick up the tickets from the match ground . The Winner(s) will receive these instructions or the Prize at least 2 working days prior to the Event.
Vitality will not be liable for any lost Prize(s) in the event that the Winner(s) have changed their email address, home address and/or telephone number, and failed to notify Vitality of this change.
The Winner(s) and their guest, if applicable, must sit on AFC Bournemouth’s side of the stadium on the day of the Event. The Winner(s) and their guest will be responsible for covering any costs associated with enjoying the Prize including, but not limited to, food, merchandise and travel costs.
The Prize is subject to AFC Bournemouth’s standard ticketing terms and conditions, available on AFC Bournemouth’s website. Please note that the Event may be subject to changes beyond the reasonable control of Vitality.
All personal data of Eligible Participants will be held and used for the purposes of the Prize Draw by Vitality in accordance with Vitality’s privacy notice (which can be found on vitality.co.uk/privacy/ or if not, requested from Vitality).
Vitality may also record photos and videos at the Event. By accepting the Prize, the Winner(s) and their guests are deemed to consent to Vitality using their images in post-promotion publicity, including but not limited to the Vitality social media pages and any other Vitality marketing communication. The Winner(s) can withdraw their consent any time by writing to Vitality.
Eligible Participants are deemed to have accepted and agreed to be bound by these Terms and Conditions upon entry. Vitality reserves the right to refuse entry or refuse to award the Prize to anyone in breach of these Terms and Conditions.
Any decisions made by Vitality over eligibility or otherwise in relation to this Prize Draw are final and binding and no correspondence will be entered into
Vitality must either publish or make available information which indicates that a valid award took place. To comply with this obligation Vitality will provide the surname and county of the Winner(s) to anyone who writes to Vitality Marketing Department, 5th Floor, Eighty Strand, London WC2R 0DT within one month after the end of the Prize Draw Period.
Vitality accepts no responsibility for entries lost, stolen or unsuccessfully completed due to a technical fault, technical malfunction, computer hardware or software failure, satellite, network or server failure of any kind.
Vitality is not liable to any person in any way in relation to this Prize Draw, except for any liability that cannot be excluded by law. Vitality will not be responsible or liable to compensate the Winner(s) for any loss, damage, personal injury or death occurring as a result of taking up the Prize. The statutory rights of Eligible Participants are not affected.
Vitality reserves the right to vary, extend or withdraw the Prize Draw at any time, including in the event of fraudulent behaviour.
Vitality reserves the right to substitute the Prize with another prize of similar value in the event the Prize is unavailable due to circumstances beyond the Vitality’s reasonable control.
Subject to clause 21, the Prize is not transferable, non-exchangeable and cannot be redeemed for cash.
Vitality reserves the right to amend these Terms and Conditions on the provision of written notice or via Vitality’s website.
The Prize Draw and these Terms and Conditions are governed by English law and Eligible Participants submit to the jurisdiction of the English and Welsh courts and tribunals with regard to any dispute or claim arising under these Terms and Conditions.
The Promoter of this Prize Draw is Vitality.
