Terms and Conditions

Purpose of the study
Vitality believes in the power of information to support better health. The purpose of this study is to elicit feedback from members on a new Health Review designed to better inform them about their health risks, and to provide more targeted feedback on how to improve their health.

The study consists of questions spanning different dimensions of health and wellbeing and will take approximately 10-15 minutes to complete. There will be no post-study follow up with participating members.

Criteria for selection
Participation in the research study is open to persons who have an in-force Vitality Health, Life or Invest policy and a wearable fitness device connected to the Vitality programme.

Participating in the NEXT Frontier research study
Participation in the NEXT Frontier research study is open to all Vitality UK members who meet the selection criteria outlined in these Terms and Conditions. Participation is by invitation only, voluntary, and free of charge, and will not advantage or disadvantage the member in any way in relation to the Vitality product(s) that they hold.

By clicking on the invitation link and downloading the NEXT Frontier application, members consent to taking part in the research study and having their data used, in anonymised form, for the purpose of this study. Members can withdraw their consent at any time, without prejudice or disadvantage.

Incentives for taking part in the NEXT Frontier research study
As a thank you for participation, Vitality will commit to plant one tree for each survey completed as part of the NEXT Frontier research study. Participants will not be able to complete more than one survey for the purpose of this incentive. Trees will be planted by donation to One Tree Planted (https://onetreeplanted.org/), a non-profit organisation focused on global reforestation.

In addition, participants will have the opportunity to win one of three Apple Watch Series 7 devices, being the 41mm Midnight Aluminium Case with Sport Band variant with GPS connectivity, valued at £369 each. The Apple Watches will be drawn at random on completion of the research study, expected to be on or before 30 November 2022. The exact date will be determined by participation in the study. Winners will be notified by email using the email address registered with Vitality.

Confidentiality and data protection
Taking part in the NEXT Frontier research study is free and participation is completely voluntary. By clicking on the invitation link, downloading the NEXT Frontier application, and submitting responses, participants provide affirmative consent to the use of their data, in anonymised form, for the purposes of the research study. Consent for participating in the study can be withdrawn at any time by emailing data.protection@vitality.co.uk, and including “NEXT Frontier research study” in the subject heading.

Vitality commits to the following principles regarding the use of data collected for the purposes of the NEXT Frontier research study:

No personally identifiable information will be collected or stored as part of the NEXT Frontier research study.
No information will be shared on a named basis with any party, including the academic partners of the study.
Responses will only be used to achieve the stated purpose of the NEXT Frontier research study.
Participants will not be exposed to marketing activity because of their participation in the NEXT Frontier research study.

Data sharing
Vitality has partnered with academics at the Anderson School of Management at the University of California, Los Angeles, to assist with study validation, data analysis, and reporting of results for the NEXT Frontier research study. No personally identifiable information will be shared with the academic partners.

Data will not be shared with any other parties.

Outcomes of the NEXT Frontier research study
The results of the NEXT Frontier research study will be used to enhance the experience of Vitality’s current health risk assessment, as made available to members through the Vitality Programme. In addition, the academic partner may utilise the data, in anonymous form, to report on the importance of health information in motivating health behaviour change among individuals.

Ending your participation in the NEXT Frontier research study
Consent for participating in the study can be withdrawn at any time by emailing data.protection@vitality.co.uk, and including “NEXT Frontier research study” in the subject heading.

Vitality reserves the right to end the NEXT Frontier research study at any time.

Privacy notice

Privacy notice overview
Vitality Corporate Services Ltd (“Vitality”) commissions the NEXT Frontier research study in collaboration with the Anderson School of Management at the University of California, Los Angeles.

Vitality is a data controller and commissions the study, capturing participant responses and performing analytical and research functions associated with the study. The Anderson School of Management at the University of California, Los Angeles is a data processor, providing independent validation of the study and undertaking supporting research functions.

This Privacy Notice explains how we will use the information you give to us whilst completing the NEXT Frontier research study.

Information collected during the NEXT Frontier Research study
Taking part in the study is voluntary. We will collect and store information about you if you download the NEXT Frontier application and complete the questionnaire. The type of information we collect includes psychographic and demographic information; information about your lifestyle behaviours; and information on your physical and mental health. We will not collect any personally identifiable information, such as your name and surname, full date of birth, and email address.

Following your completion of the study, we may match your responses to your health and wellbeing information that we collect through the Vitality Programme.

Using your information
The information collected through the NEXT Frontier research study will be used to enhance the experience of Vitality’s current health risk assessment, as described in the accompanying terms and conditions. Your anonymised information will be combined with information collected from other participants to the study, meaning that your data will never be used in isolation or be identifiable in the data set.

Vitality has chosen to offer an incentive for participation in the NEXT Frontier research study. To fulfil the guaranteed incentive and to advise winners of the prize draw, Vitality will use your email address, as registered with Vitality, for the purpose of the incentives. Downloading the NEXT Frontier application generates a unique ID which we link to your email invitation. We only receive a notification through the ID that the study is completed, allowing us to confirm the donation to One Tree to the participant and notifying the winner(s) of the Apple Watch Series 7.

We may provide your email address to One Tree Planted, the charity partner selected for this initiative (as outlined in the accompanying terms and conditions), for the purposes of sending you a certificate in relation to the tree planted in your name.

Accessing information we hold about you in relation to the NEXT Frontier research study
As no personally identifiable information is collected from you as part of the NEXT Frontier research study, it is not possible to identify the survey responses that you have provided. As a result, we are not able to provide you with a copy of the information that you have submitted through this study.

Raising complaints and withdrawing consent
Consent for participating in the study can be withdrawn at any time by emailing data.protection@vitality.co.uk, and including “NEXT Frontier research study” in the subject heading.

Incentive terms and conditions

For the purpose of this prize draw:
1. The NEXT Frontier research study refers to the survey delivered via the NEXT Frontier application that is made available to certain Vitality UK members by direct invitation.
2. The Study Period refers to the dates over which the NEXT Frontier research study will be run, commencing 12 October 2022 and ending on or before 30 November 2022.
3. Draw Date is expected to be on or before 30 November 2022 at the conclusion of the NEXT Frontier research study.
4. Eligible Participant means any person who has been invited to take part in the NEXT Frontier research study and has submitted a completed survey through the NEXT Frontier application.
5. Prize means a choice of either one of three Apple Watch Series 7 devices, being the 41mm Midnight Aluminium Case with Sport Band variant with GPS connectivity, valued at £369 each; or one of three £300 Amazon.co.uk Gift Cards (Restrictions apply, see www.amazon.co.uk/gc-legal)
6. Promoter means Vitality Corporate Services Limited (company number 05933141) of 3 More London Riverside, London SE1 2AQ trading as VitalityHealth, VitalityLife, VitalityCar and VitalityInvest.
The prize draw is open to all Eligible Participants who comply with these terms and conditions and only one entry per Eligible Participant is permitted. The Promoter reserves the right to refuse to award the Prize to anyone in breach of these terms and conditions.
The prize draw is free to enter and no purchase is necessary.
The winners of the Prize Draw will be drawn at random by Vitality in the presence of an independent witness.
Eligible Participants who complete the NEXT Frontier research study during the Study Period will be entered into a prize draw and the winners will be drawn at random by the Promoter on the Draw Date.
The Promoter accepts no responsibility for entries lost, stolen or unsuccessfully completed due to a technical fault, technical malfunction, computer hardware or software failure, satellite, network or server failure of any kind.
The winners will be contacted by email, using the members policy details, within 5 working days of the Draw Date. This will include details of how to claim the Prize. In the event that we fail to obtain confirmation from the winner(s), or the winner(s) refuses the Prize or for whatever reason cannot accept the Prize, the original winner(s) will forfeit the Prize immediately, and an alternative winner(s) will be drawn and notified in accordance with these terms and conditions.
The decision of the Promoter regarding any aspect of the prize draw is final and binding and no correspondence will be entered into about it.
Other than as set out in clause 9, the Prize is not transferable, nonexchangeable and cannot be redeemed for cash.
The Promoter reserves the right to substitute the Prize with another prize of similar value in the event the Prize is unavailable due to circumstances beyond the Promoter’s reasonable control.
The Promoter reserves the right to void, cancel, suspend, or amend the Prize where it becomes necessary to do so including in the event of fraudulent behaviour.
Insofar as is permitted by law, the Promoter, its agents or distributors will not in any circumstances be responsible or liable to compensate the winner(s) or accept any liability for any loss, damage, personal injury or death occurring as a result of taking up the Prize except where it is caused by the negligence of the Promoter, its agents or distributors or that of their employees. The statutory rights of Eligible Participants are not affected.
The Promoter reserves the right to amend these terms and conditions at any time by providing written notice to Eligible Participants.
The prize draw will be governed by English law and Eligible Participants submit to the jurisdiction of the English courts.
The surname and county of the prize draw winners will be made available on request by writing to the Marketing department, Vitality Corporate Services Limited, 4th Floor, 70 Gracechurch Street, London, EC3V 0XL.
