Terms and Conditions
Business Healthcare £100 Waitrose & Partners Offer
1. This Offer is available to all Eligible Participants who comply with these terms and conditions.
2. For the purposes of these terms and conditions:
(a) “Business Healthcare Plan" means a general insurance contract for the provision of private medical insurance services, underwritten by Vitality Health Limited and administered by Vitality, which is sold to an SME;
(b) “Personal Healthcare Plan” means a general insurance contract for the provision of private medical insurance services sold to an individual in their personal capacity, underwritten by Vitality Health Limited, and administered by Vitality through VitalityHealth;
(c) “Eligible Participant” means either: (i) an individual who has not held a VitalityHealth Plan in the twelve months prior to the start of the Offer Period and who is sent an email from Vitality inviting them to take up the Offer (an “Eligible PHC Participant”); or (ii) the group secretary of an SME which has not held a VitalityHealth Plan in the twelve months prior to the start of the Offer Period and who is sent an email from Vitality inviting that SME to take up the Offer (an “Eligible BHC Participant”);
(d) “Keyed and Activated” means a VitalityHealth Plan in respect of which Vitality has accepted the risk and the start date has been reached;
(e) “Offer" means the offer of a Voucher to those Eligible Participants who qualify to receive it in accordance with these terms and conditions;
(f) “Offer Period” means the 1st March 2022 to midnight 31st December 2023;
(g) “SME” means a small to medium size business employing 1-249 (inclusive) people;
(h) “Vitality” means Vitality Corporate Services Limited (registered number 05933141) of 3 More London Riverside, London SE1 2AQ;
(i) “VitalityHealth” is a trading name of Vitality Corporate Services Limited;
(j) “VitalityHealth Plan” means either a Business Healthcare Plan or a Personal Healthcare Plan;
and (k) “Voucher” means a Waitrose voucher to the value of £100.
3. Eligible PHC Participants will be entitled to receive a Voucher if:
(a) they submit an application for a new Personal Healthcare plan during the Offer Period and (subject to clause 5) that plan is then Keyed and Activated before the end of the Offer Period; and
(b) Vitality has collected the premiums for the first three months of that plan.
4. Eligible BHC Participants will be entitled to receive a Voucher if:
(a) the SME for which the Eligible BHC Participant is the designated group secretary submits an application for a new Business Healthcare plan during the Offer Period and (subject to clause 5) that plan is then Keyed and Activated before the end of the Offer Period; and
(b) Vitality has collected the premiums for the first three months of that plan.
5. If an Eligible Participant submits an application for a new VitalityHealth Plan during the Offer Period but that plan is not Keyed and Activated before the end of the Offer Period, the Eligible Participant will still be entitled to receive a Voucher if:
(a) their plan is Keyed and Activated on or before 31st January 2024; and
(b) Vitality has collected the premiums for the first three months of that plan.
6. Vitality will notify Eligible Participants who have qualified for the Offer by email before the end of the fourth month from the start date of their VitalityHealth plan. The Eligible Participant must then redeem the Voucher within 30 days of receipt of this email, following which the Voucher will be emailed to the Eligible Participant within a further 45 days of the date on which it was redeemed.
7. Vitality will contact the Eligible Participants using the email address Vitality has on record for them. Vouchers which are not received because the Eligible Participant has provided Vitality with an incorrect email address will not be re-sent. To ensure Vouchers are delivered, Eligible Participants are advised to modify their marketing preferences on the “Vitality Member Zone” to receive all communication from Vitality (which can be done via the Administration tab at the top of the Member Zone home page).
8. All Vouchers will be valid for twelve 12 months from their date of issue.
9. Vouchers are not transferable, are non-exchangeable and cannot be redeemed for cash.
10. Vitality reserves the right to substitute the Voucher with another item of similar value in the event the Voucher is unavailable due to circumstances beyond Vitality’s reasonable control.
11. Any decisions made by Vitality over eligibility or otherwise in relation to the Offer are final, will not be open to dispute and no correspondence will be entered into.
12. Vitality is not liable to any person in any way in relation to the Offer, except for any liability that cannot be excluded by law.
13. Vitality reserves the right to amend or withdraw this Offer at any time, including in the event of fraudulent behaviour by any Eligible Participant.
14. Vitality reserves the right to amend these terms and conditions on the provision of written notice or via Vitality’s website.
15. These terms and conditions are governed by English law and the parties agree to submit to the exclusive jurisdiction of English courts and tribunals with regard to any dispute or claim arising under them.
